Carcinoid tumour at base of appendix, causing appendix perforation and Generalised Peritonitis.

A 23 y old female presented with abdominal pain for two days. Analysis of pain revealed that on the first day the pain started peri umbilically and later shifted to right iliac fossa, suggestive of appendicitis. On the next day, the pain worsened and she came to the A&E, in which peritonitis involving the lower half of abdomen was diagnosed click ally. Classic features like tenderness, and rigidity were there and rebound tenderness was elicited by indirect method, asking the patient to try to breathe deep, which caused increased degree of pain.

After the relevant investigations and preparation, an Exploratory Laparotomy was performed. Perforated appendix with a firm , less than 1 cm nodule at the base of the appendix was noted. A Right Hemicolectomy and Ileo-Colic anastomosis was performed and the peritoneal cavity was thoroughly washed. Patient recovered completely. Histology came as a Carcinoid tumour. Further investigations excluded the liver secondaries. Patient was referred to the Oncology Department.
